Aberdeen City Health & Social Care Partnership is requesting notes of interest from providers who would be able to provide interim placements for adults in care homes in Aberdeen. Interim capacity can be defined as the interim arrangements made to provide an appropriate level of care and support to achieve personal outcomes in a place for a period of time.
This provision includes interim arrangements for people either from hospital or community who due to the level of care and support needed to meet their outcomes, require to continue their lives in a care home environment. Interim arrangements allow them to do this whilst they wait for capacity to become available in the care home of their choosing.
It also includes interim arrangements for the delivery of appropriate accommodation and care and support to meet their outcomes as they work towards gaining better independence on the journey back to better health and wellbeing at home.
